Embark on a transformative journey from August 22nd to December 6th, as you play a pivotal role in driving healthy change across our dedicated community clinics in Houston, Texas. As a Health Advocate Intern, you'll provide impactful case management services, focusing on the social determinants of health and facilitating meaningful connections between patients and vital community resources. By documenting endeavors in the sophisticated REACH client management database, your work will echo across our diverse communities, ensuring health equity and advocacy at every step.

Core Responsibilities

  • Devote over 8 hours per week to volunteer advocacy over a semester, immersing yourself in community service.
  • Screen and evaluate clients' needs related to social determinants of health, employing cutting-edge tools and methodologies.
  • Conduct real-time research to provide optimal referrals and connections to essential social service resources.
  • Collaborate with families to craft effective plans for accessible service connections and overcome linkage barriers.
  • Maintain professional and accessible communication that aligns with Legacy's values when following up on client plans.

Essential Qualifications

  • Bilingual proficiency, especially English/Spanish, is preferred, though proficiency in any language is a plus.
  • Current enrollment in a post-secondary program at an affiliated university is essential, or ability to facilitate affiliation documentation if not currently partnered.
  • Educational pursuit in fields such as Health Care, Social Work, Nonprofit Management, Sociology, Public Health, or related areas is beneficial.
  • Preferred background in community service, employment, or coursework related to health and economic disparities, and social justice issues.
  • Ability to meet the EPIC proficiency standards set by Legacy Community Health.
